Major Medical Insurance

B.A.E.S.S. provides major medical insurance through for all regular full-time employees only.  Employees may elect to participate in the company sponsored plan or not.  If they choose not to participate, the company will not reimburse the employee for their medical insurance costs nor will they adjust their pay accordingly.  The company will pay 100% of the cost of the lowest HMO quoted by the provider annually for the employee only.  Employees who wish to add benefits or to select another carrier under the provider menu and/or add family members, will be charged the difference between the lowest cost HMO and the insurance of their choice.

Purpose

  • To allow eligible employees to enjoy the benefits of major medical insurance without having to pay for it individually.

Eligibility:  Regular Full-time Employees.  The first day of the month following two full calendar months of employment.

Employee Responsibilities

  • Complete the necessary paperwork and submit it to your manager in a timely fashion.
  • Review your options annually to make the best choice for you.

Manager Responsibilities

  • Forward all completed paperwork to the main office on a timely basis.
  • Manager must ensure that employee signs appropriate form if employee elects to waive health coverage.

Executive Responsibilities

  • Analyze the insurance options on an annual basis and determine what insurance program the company will have each year.
  • Provide sufficient information to employees so they can make an educated decision regarding their major medical insurance coverage.
  • Provide the necessary forms to each manager so that his/her employees have adequate time to make their decision and complete their paperwork.
  • Be sure all forms are transmitted to the carriers in a timely fashion.
